Durable Power of Attorney: A durable power of attorney allows another individual to do legal and financial things on your behalf, such as pay bills and get money out of your checking account. It can become effective upon signing or when you become incapacitated and can be valid for as long as you want. Fee: $75.00 Form: Durable Power of Attorney Information Sheet
Medical Power of Attorney: A medical power of attorney allows another individual to make health care and medical decisions for you if you become incapacitated. This includes withdrawing life support and organ donations. Fee: $75.00 Form: Medical Power of Attorney Information Sheet
Temporary Custody Power of Attorney: This authorizes another individual to have custody of and provide reasonable care for your minor child including providing medical care and enrolling in school. (Note: you need to check with your school system to see if they will accept this for enrollment, some schools require a court ordered custody for this purpose). You can indicate how long the power of attorney is valid for. This does not go through the courts or is custody ordered by a judge. Special Power of Attorney: This gives specific powers to another individual to perform a specific action on your behalf such as selling a house or registering a vehicle. Fee: $75.00
